Companion Services
Otherwise known as Community Inclusion- Services consist of non-medical care, supervision, and socialization activities provided to an adult. This service will provide direct relation to the achievement of the Individuals goals per the support plan. A companion service is performed in the community and include activities of the Individuals choice, including their personal support plan goals. This service does not entail hands-on medical care. Services may be scheduled on a regular, or long term basis.
Personal Supports
Personal supports are services that provide an Individual in a supported living situation with four to twenty four hours a day assistance from a support worker or support workers. The support worker may live on the Individuals home or apartment sharing living expenses with the Individual. The support worker provides companionship and personal care, and may assist with or perform activities of daily living and other duties necessary to maintain the Individual in supported living. The personal support services are separate and not a replacement for the services performed by a Supported Living Coach.
Respite Care Assistance
Respite care is a service that provides supportive care and supervision to an Individual when the primary caregiver is unable to perform these duties due to a planned brief absence, an emergency absence or when the caregiver is available, but temporarily physically unable to care or supervise the Individual for a brief period.
Supported Living Coaching
SLC services provide training and assistance, in a variety of activities to support Individuals who live in their own homes or apartments. These services may include assistance with locating appropriate housing; the acquisition, retention or improvement of skills related to the activities of daily living such as personal hygiene and grooming; household chores; meal preparation; shopping; personal finances, any and all medical appointments and establishing a relationship with the Individuals doctors, as well as the social and adaptive skills necessary to enable individuals to reside on their own.
Supported Employment
SE services consists of assistance with assessments, career planning, job development, on the job training, travel skills, interpersonal communication, time management, personal hygiene, third party benefits, and any other special issues. In addition, specific training toward support goals is provided.
Consumable Medical Supplies
This service consists of managing an individuals medical supply needs. For example an Individual with regular supplies such as wipes, diapers, and seat liners, may have these items ordered through us and delivered to your front door.
